- Medical Waiver (3 notarized copies)
- Player Application (if you do not have an online account already)
- Team Commitment Form
- Uniform Order
For players new to Mecklenburg Union FC, a copy of a birth certificate is also required.
Please send the above documentation and a one inch by one inch photo (in color, on photo paper, no hats, no sunglasses) to the following address by Wednesday, June 4th:
